We could never put into words what this year has meant to our family. Our instincts immediately told us Heroic was the place for Isaac, you were what we had been looking for . Isaac had been a resident at a group home for a number of years. Regrettably, his mental health began to decline, leading to increased isolation and significant self-harming behaviours.
The multidisciplinary team identified that the shared living environment was no longer conducive to Isaac’s well-being, necessitating a change in his living arrangements. Isaac’s daily routine had become increasingly restricted. He confined himself to his bedroom, consumed only finger foods in solitude, and showed diminished interest in outdoor activities, except for car rides. His parents were unable to engage with him in a personal setting as he was uncomfortable with their presence in the house. Their interactions were limited to car journeys, and they had not been able to establish close contact with him for years. Isaac’s world seemed to be shrinking.
In January, Isaac’s care was transitioned to Heroic Care, and he moved into a newly constructed bungalow for a fresh start. As part of the transition process, comprehensive information was collected, and an in-depth meeting was held with his new team, which included his parents.
The meeting aimed to establish a foundation for Isaac’s future by setting realistic and achievable goals. His parents understood that this would be a gradual process and anticipated it would take some time for Isaac’s world to ‘open again’.
In consultation with his parents, the team agreed on the following goals for Isaac:
· Consuming nutritious, homemade meals at the kitchen table.
· Engaging in walks as opposed to solely car journeys.
· Utilizing all areas of his new home, rather than confining himself to his bedroom.
Remarkably, within the first week, Isaac began eating at the kitchen table and enjoying walks in local parks, which quickly became part of his daily routine.
Within a month, Isaac had visited a supermarket for his weekly shopping, enjoyed meals at a pub, and even attended a successful dentist appointment.
The most significant achievement to date is that Isaac’s parents have been able to visit his new home, share a cup of tea, and have physical contact with him. He has remained calm and content during these interactions. These were unforeseen, heartwarming developments that occurred sooner than expected.
The successful transition can be attributed to a dedicated and consistent staff team, effective communication, invaluable contributions from his loving parents, clear goal setting, and comprehensive support plans/risk assessments. We are highly optimistic about Isaac’s future within Heroic Care and eagerly anticipate his continued progress and achievements.
